Edison worker dies in Industrial accident

On Thursday, April 25, 2013, about 7:45 AM, officers from the Menifee Police Department responded to the area of Murrieta Rd., north of Park City Ave, in the city of Menifee. Upon the arrival of officers, employees from Southern California Edison were attempting to treat a fellow employee who sustained significant injuries from a high voltage line. Cal Fire and AMR personnel arrived within minutes and were unable to resuscitate the victim. As a result, the victim was pronounced deceased at the scene.

CAL OSHA and Southern California Edison are investigating the incident. A section of Murrieta Rd from Ridgemoor Ave. to Park City Ave. was closed to both vehicle and pedestrian traffic. Early in this investigation it was determined this incident was not related to a massive power outage that occurred in the city of Menifee on the prior day. Fatal accident on the 57 freeway

Orange County’s former chief executive officer ran into a disabled vehicle while driving on the 57 Freeway, a crash that killed two people, the California Highway Patrol reported.

According to a preliminary report, Thomas Mauk, 69, rear-ended a disabled vehicle on the 57 Freeway near Diamond Bar late Sunday.

Anubhai Patel, 67, and his wife, Shardaben Patel, 68, were inside the vehicle at the time of the crash. The Anaheim couple died from their injuries.

Police said Mauk and his wife, Denise, both from Whittier, were southbound on the freeway when their 2008 Cadillac Escalade slammed into the back of the 2006 Toyota Prius north of Pathfinder Road.

The Prius had been reported as disabled in the No. 3 lane just minutes earlier, according to a CHP report.

Mauk and his wife suffered minor injuries and neither was arrested after the crash, which remains under investigation.

Mauk resigned from his Orange County post last summer amid heavy criticism on how county executives handled an internal investigation into a fellow executive’s alleged misconduct.

Fatal Traffic Collision in Hesperia

 

On April 23, 2013 at about 2:50 p.m, Deputies from the Hesperia Station and units from the California Highway Patrol along with rescue personnel from the San Bernardino County Fire Department responded to a report of a two vehicle, head on traffic collision in the area of Hwy 138 and Little Horsethief Ranch Rd. in Hesperia. It was reported that the two vehicles had collided head on and that there were serious injuries to several parties. Rescue personnel arrived on scene and discovered that the male driver of a 1991 Ford Explorer was trapped inside his vehicle and suffered fatal injuries upon impact. The driver was later identified as Robert E. Miller, age 57 of Victorville. A male passenger from that same vehicle was suffering from injuries, not life threatening.

Three occupants from the second vehicle, a 1999 Toyota minivan were also seriously injured, one with life threatening head injuries. Rescue personnel extricated the victims from their vehicles and all were transported to Arrowhead Regional Medical Center by both air and ground transportation.

The Hesperia Station Major Accident Investigation Team (MAIT) responded to conduct the investigation. It was determined that for unknown reasons the driver of the Ford Explorer who was traveling east on Hwy 138 at an undetermined speed, crossed over into the westbound lanes and collided head on into the Toyota minivan as it traveled west on Hwy 138.

Hwy 138 was closed in both directions between Summit Valley Rd. and Hwy 173 until about 1930 hours while investigators processed the scene for evidence. At this time the exact cause of the collision and why the driver of the Ford Explorer crossed into the westbound lane is still under investigation by the Hesperia Station Traffic Division MAIT Investigators.

Bicyclist killed in Carlsbad crash

A 45-year-old bicyclist killed Sunday morning after he was struck by a vehicle in Carlsbad has been identified as Eric Ringdahl, the San Diego Medical Examiner’s Office said.

Mr. Ringdahl was a married  and lived with his wife and children in Carlsbad. On April 21, 2013, he, was headed north on El Camino Real just passed La Costa Avenue with another bicyclist when he was struck by a northbound vehicle.  911 calls for medical assistance were made by multiple parties. Coachella Festival Worker Killed in Traffic Collision

On Saturday, April 20, 2013, approximately 7:58 AM, officers from the La Quinta Police Department responded to the intersection of Avenue 52 and Madison Street, regarding a major injury traffic collision between a motorcycle and an automobile. Woman killed in crash with military Humvee

A 26-year-old Yucca Valley motorist was fatally injured in a collision with a military Humvee in Joshua Tree, according to San Bernardino County coroner’s officials.

About 3:30 p.m. Friday, 19-year-old Juan Lira, a U.S. Marine from Oceanside, was driving a Humvee east on Twentynine Palms Highway. For unknown reasons, the California Highway Patrol reports, Lira crossed into opposing traffic and hit Thrawl’s westbound Focus.

The accident happened about one mile east of White Feather Road in Joshua Tree. Thrawl was pronounced dead at the scene at 3:45 p.m. Lira was not hurt. Capt. Nick Mannweiler, spokesman for the Twentynine Palms Marine base, said the Humvee came from a Camp Pendleton-based unit.

California Highway Patrol officers are investigating the accident.

Bus Crashes near Yosemite: More than a Dozen Injured

As reported in the Los Angeles Times the charter tour bus that crashed Saturday evening outside Yosemite National Park, injuring more than a dozen people, was traveling at an unsafe speed when the driver lost control, according to the California Highway Patrol.

The bus was leaving Yosemite on California 41, or Wawona Road, about 6 p.m., when it veered off the roadway and angled up a dirt embankment, according to a CHP report. As the driver attempted to regain control of the vehicle, the bus crossed both lanes of traffic and struck a tree.

“If the tree wasn’t there to stop the bus, it would have continued down a ravine,” CHP Patrol Sgt. Edward Green told the Fresno Bee. The bus came to a stop with its two rear tires in the air.

Seventeen people were aboard the vehicle; 15 passengers and a tour guide sustained minor to moderate injuries in the crash, authorities said. The driver was uninjured.

The speed limit for the road is 55 mph, but several turns in the roadway require slowing, according to Scott Jobinger, a CHP spokesman. The accident occurred about a mile north of local road 630 and about 40 miles south of the park.

The bus was operated by Seven Happiness Tour and Charter Inc. in Burlingame, Calif., Jobinger said.

Adam Wang, a manager with Seven Happiness, said that the tour began Saturday morning in the Bay Area, and after a tour of the park, the bus was headed to Fresno for the night. It was to return to Burlingame on Sunday. Seven Happiness runs about two charters to Yosemite each week.

Intoxication was not a factor in the crash, the CHP said. A vehicle inspection will be performed on the bus to determine if there were any mechanical issue involved in the accident.

Coca Cola Bottling Plant Fatal Accident

A 58-year-old Mission Viejo man was killed Friday at a Coca-Cola water bottling plant in Anaheim, the victim of an apparent industrial accident, according to the coroner’s office.

Alberto Gonzalez died Friday after the accident at the plant on 2121 E. Winston Road in southwest Anaheim, according to the coroner’s office. Fire department personnel responded to the plant around 1:30 p.m. and pronounced Gonzalez soon after they arrived, according to police Sgt. Bob Dunn.

“The initial investigation revealed the victim, an employee of Coca Cola, was attempting to make repairs or adjustments to a machine used to load cases of beverages onto pallets,” Dunn said. “While the victim was inside the machine, it activated, and fatally injured the victim.”

An autopsy is scheduled for early next week. The Anaheim Police Department is investigating.

San Bernardino man in wheelchair dies after being hit by car

San Bernardino Police Department officers responded to a 911 call at 8:10 p.m. for an auto versus a pedestrian in a wheelchair at the intersection of Golden Avenue and Date Street, in the city of San Bernardino. Officers arrived on scene and found a 55 year old Black male with trauma to the head and upper body. Paramedics transported him to Loma Linda University Medical Center where he was later pronounced dead at 8:51 PM. An autopsy will be conducted to determine the cause of death. The name of the decedent will be released once next of kin has been notified. The San Bernardino Police Department is investigating the incident.

2 Dead, 40 Injured in Charter Bus Crash

Two people were killed and dozens rushed to hospitals after a charter bus bound for Oklahoma overturned on a Dallas-area turnpike Thursday morning.

Approximately 40 elderly people were trapped in the chartered bus, which was transporting the retired people to a gambling outing at the Choctaw casino in Oklahoma, according to USA Today.

“There was obviously a lot of pain,” a witness who helped pull victims from the wreckage told NBC DFW. “You had to start with whoever was on top, try to untangle them. People were just screaming.”

The bus belongs to the Cardinal Coach LInes of Mansfield, Texas.

The stretch of State Highway 161, also known as the President George Bush Turnpike, is located approximately 15 miles outside Dallas.

The driver is 65-year-old Loyd Rieve, the Texas Department of Public Safety said. He was investigated in a 1998 accident, but a grand jury declined to indict him on a charge of criminally negligent homicide.

The bus driver was injured in the Thursday accident, which occurred on the President George Bush Turnpike in Irving. It involved only one vehicle.

Department of Public Safety spokesman Sgt. Lonny Haschel told news agencies that the bus went off the right side of the road, hit a barrier, swerved left across lanes of traffic and struck a concrete median in a grassy area.

“It rode up on top of that concrete barrier, and then rolled on its right hand side, where it came to rest,” Haschel said.

Investigators are looking at several factors, including the vehicle and physical evidence on the road.

The National Transportation Safety Board said it has sent two investigators to the scene of the wreck.
