CHP Reports Alcohol played role in fatal Willow Creek bus crash

An unidentified 19-year-old woman was killed and seven other people were injured when a bus drove off a steep embankment on State Route 96 late Friday night, according to a California Highway Patrol press release issued Saturday.

Around 9:50 p.m. a 1993 GMC former school bus driven by Lorenzo Jackson, 42, of Washington state, drove off the right of the highway for unknown reasons approximately four miles north of State Route 299, according to the release. The bus rolled a short distance down a steep embankment, and came to rest against some trees.

Jackson broke his leg exiting the bus, according to the release. Two other passengers exited the bus and remained nearby while two passengers walked up to State Route 96. Three people remained in the bus, including a woman who was pronounced dead at the scene by paramedics.

Deputy Coroner Roy Horton said the woman’s identity is currently unknown, and that the passengers on the bus did not know very much about each other.

During investigation of the crash, it was determined that both Jackson and 35-year-old Joseph Forcier of Florida had driven the bus under suspicion of alcohol intoxication prior to and during the collision, according to the release.

Both men were arrested and remain at medical facilities to be treated for their injuries. Forcier received major head trauma and was transported to Mad River Community Hospital, according to the release.

Five other accident victims were transported to St. Joseph and Mad River Hospitals.  The deceased victim’s name has not been released.

Woman Who Died Trying To Cross Street ID’d

A female pedestrian struck by two vehicles as she crossed the street in the Mountain View area has been identified.

The incident occurred just after 9 p.m. Wednesday in the 4300 block of Oceanview Boulevard near San Pasqual Street, according to police.

Police said 64-year-old Catalina Torres Velazquez had just exited a bus and was trying to cross the street when she stepped in front of a Nissan Frontier pickup truck driving westbound on Oceanview Boulevard.

Velazquez was thrown over the truck and onto the pavement. She landed in the eastbound lane of traffic, where she was hit over by an Acura, police said.

Medics took Velazquez to UCSD Medical Center in Hillcrest. She was pronounced dead shortly after arrival.

The drivers of both cars did stop and worked with police to reconstruct the crash,  according to 10News .

Police say that speeding and alcohol were not factors in the woman’s death.

3 hurt as Muni trolley knocks down live wires

Three people were injured when a Muni trolley bus snapped live electrical lines at Market at Fifth streets during the Monday evening rush hour, authorities said.

The 4:07 p.m. incident forced the closure of Market between Third and Eighth streets and part of Fifth Street, snarling downtown traffic. Crews were unable to shut off live downed electrical lines until just before 5 p.m., and the street was expected to remain closed at least until 7 p.m., authorities said.

Witnesses described hearing a loud, explosive noise when the accident happened.

One pedestrian, a woman in her 20s, was hit by one of the wires, said Fire Department spokesman Mindy Talmadge. She and another person were taken to St. Francis Memorial Hospital. The nature of their injuries was unclear, but Talmadge said neither was badly hurt.

Paul Rose, a Muni spokesman, said a third person had also been taken to a hospital.

One of the victims, a pedestrian who was apparently a tourist, was helped by a San Francisco man after being hit by one of the wires.

“I heard a pop and saw one man under the wire,” said Hilton Holcomb, 57, who was crossing Fifth at the time of the incident. “I saw sparks and the fire coming through. I pulled him to the side of the road.”

Holcomb said the man was conscious but was shaking and complaining that he felt like his hands were on fire.

“I’ve seen these wires fall before,” Holcomb said, “but I’ve never seen one with explosions and fire.”

Initial reports were that the “hub” mounted on the 5-Fulton trolley’s pole that includes a transformer became detached and snapped live wires as it fell from the bus, authorities said. The cause of the June 4 crash remains under investigation by the San Francisco Police Department.

Fatal Motorcycle Crashes Showed No Improvement Last Year

Preliminary data from all 50 states and the District of Columbia showed no improvement in motorcycle fatalities in 2011 despite a strengthening economy and rising gas prices, reports a new study released by the Governors Highway Safety Association (GHSA).

Projecting from state fatality counts contributed during the first nine months of 2011, the GHSA says that motorcycle fatalities remained at 4,500 last year, the same level as in 2010. The study tracks statistics from January through September 2011.

The year has been kind to most other drivers, however, with The National Highway Traffic Safety Administration (NHTSA) projecting that general motor vehicle fatalities declined by 1.7 percent in 2011, reaching their lowest levels since the late 1940s. Motorcycles seem to be one of the only highway safety areas in which safety statistics have not improved.

Motorcyclist fatalities have decreased in twenty-three states between 2010 and 2011. In Connecticut, the GHSA report says, motorcycle deaths declined by 37 percent, North Carolina reported a drop of 21 percent and New York a drop of 16 percent than the previous year.

The reduction in fatal crashes in these states owes its success to better enforcement, education and engineering, says Barbara J. Fiala, commissioner of the New York State Department of Motor Vehicles and GTSC chair, in a statement. “In New York, we are educating motorists to watch for motorcycles, riders to wear bright protective gear to make themselves more visible, and law enforcement officers on conducting efficient and effective motorcycle checks.”

However, that achievement is leveled by the 26 states and the District of Columbia which report a surge of up to 26 percent in motorcyclist deaths. South Carolina reported the biggest leap in human losses, while motorcycle fatalities in Texas and California rose by 16 percent and 10 percent between 2011 and 2010, respectively.

 

A silent and surprising contributor to this increase may be the economy, says Dr. James Hedlund of the GHSA, who authored the study. He compared gas prices, motorcycle registrations, and motorcyclist fatality trends since 1976, and found that gas prices are directly related to deaths. According to the Association and also the Motorcycle Safety Foundation, the costs of filling up a tank of gas may make the fuel efficiency of motorcycles more attractive.

Another factor is the economy, which began an upward trajectory in 2011, increasing the disposable income in people’s pockets. A statement from the Insurance Information Institute (I.I.I) cites the Motorcycle Industry Council (MIC) estimate that the percentage of U.S. households with at least one bike rose from 5.4 percent in 2003 to 6.8 percent in 2008.

Helmet laws, the NHTSA’s highest-rated safety countermeasure, have decreased across the United States. Only 19 states currently require riders to wear helmets, whereas in 1997, 26 states made it law. The lowest motorcyclist fatality rates since 1975 occurred during 1996 and 1997, GHSA study statistics indicate.

In a statement, Troy Costales, GHSA chairman, expressed his disappointment with the declining progress of motorcycle safety, saying, “The study notes, the strengthening economy, high gas prices, and the lack of all-rider helmet laws leave me concerned about the final numbers for 2011 and 2012. Every motorcyclist deserves to arrive at their destination safely. These fatality figures represent real people – they’re family, friends and neighbors.”

As far as its impact on individual motorcycle-insurance policyholders, the I.I.I says that since the fatality rate stayed steady from 2010 to 2011, there may be no change on current insurance rates.

“When underwriting a policy, a company will look for a mature and experienced operator, someone with a good driving record, whether the motorcycle is garaged, the age of the driver, and if they have any accidents, among other risks,” says Loretta Worters, I.I.I vice president, in an e-mail.

“Operating a motorcycle requires a different combination of physical and mental skills than those used in driving four-wheel vehicles,” she says, also citing vulnerability to weather hazards and road conditions as factors that underwriters take into consideration when developing rates.

In an e-mail statement, Dan Kamionkowski, Progressive Motorcycle Product Manager says of the study, “There are several factors that go into writing an insurance policy for a motorcyclist. The individual we’re insuring is largely what impacts the underwriting process. In setting overall rates, we focus on our internal claims history rather than external studies.”

The GHSA recommends concentrating on several safety areas to ensure that 2011’s dismal rates improve in future years. Helmets have been proven to be 37 percent effective at preventing fatal injuries to motorcycle operators, and even more so for passengers, says the NHTSA. Alcohol impairment, an obvious contributor to accidents, caused 29 percent of all rider fatalities in 2010.

Reduced speeding may lessen the risk of fatal crashes, even those not involving another vehicle. Rider and driver training is also important to safe motor vehicle operation. A large share of this responsibility, says the NHTSA, falls on other motorists: when motorcycles collide with vehicles, it is usually the car or truck which violates the motorcyclist’s right of way.

The warnings relayed in the report are timely, as this Memorial Day Weekend almost 100,000 motorcyclists will arrive in Washington, D.C., after nationwide holiday rally “Rolling Thunder”. Four other rallies will take place over the course of the three-day weekend.

“With so many motorcyclists on the road this month,” says the GHSA’s Costales, “It’s an opportune time to remind all motorists about this critical highway safety issue.”

1 dead, baby critical after 3-car crash in H.B.

Authorities have identified a Long Beach man killed Wednesday morning in a multi-vehicle crash that also left a 7-month-old baby in critical condition.
The driver of an orange Ford Bronco lost control of the vehicle along Bolsa Chica Street, crossed a center divider and into opposing lanes, said Lt. Gary Faust of the Huntington Beach Police Department.
Orange County Coroner’s Office officials say Abraham Perez, 30, was pronounced dead at the scene.
Three others were taken to nearby hospitals. Two of those victims suffered minor injuries, but the baby was in critical condition.
Police were called to the crash at Bolsa Chica Street and Tasman Drive just after 7:31 a.m., Faust said.
The Bronco was headed north when the driver lost control and crossed into southbound lanes, Faust said, hitting a silver Honda. A third vehicle headed south on the left lane then crashed into the wreck.
The impact of the crash caused the Bronco to turn onto its roof.

PALM SPRINGS: Fatal crash closes Gene Autry Trail

PALM SPRINGS – Emergency crews responded to a fatal crash late Thursday along Gene Autry Trail, just south of I-10. Police say two cars collided head-on. One driver, 54-year-old Joyce Stotz of Palm Springs died at the scene of the crash, according to the Riverside County Coroner’s Bureau. The 20-year-old male driver of the second vehicle amd a 17-year-old female passenger were taken to the hospital with severe injuries

About a half-mile stretch of Gene Autry Trail was shut down in both directions between I-10 and Via Escuela, just north of Vista Chino, for several hours.

Investigators say Stotz was traveling northbound on Gene Autry Trail when she drove through a patch of sand, lost control of her vehicle and drove into the southbound lanes of traffic, colliding with another vehicle. Stotz was ejected from her car.

Palm Springs Police Department are investigating the crash.

39 students injured in Wash. school bus crash

 

A school bus overturned Monday morning on a central Washington highway, injuring dozens of children, including three seriously, before coming to rest at the edge of a field, authorities said.

The bus veered off the road, overcorrected and rolled on Highway 281 just south of Quincy, an agricultural region about 120 miles east of Seattle, said Lt. Scott Martin of the Washington State Patrol.

The bus came to rest on its passenger side, and all 39 students on board suffered injuries of some kind, though most were minor scrapes, bruises and bloody noses, Martin said. Three children were in serious condition, but none of the injuries was believed to be life-threatening.

One child was airlifted to the nearby city of Moses Lake, and two were taken to a hospital in Ephrata. The rest of the students went to the local Quincy Valley Medical Center, either for treatment or to be reconnected with their families.

The driver apparently was not injured, and no other vehicle was involved.

The Quincy School District covers about 450 square miles of farmland in central Washington’s Columbia River basin, which is home to thousands of acres of tree fruit, and potato and onion fields. The district serves 2,500 students.

“We are deeply concerned about the students and the driver,” the district’s superintendent, Burton Dickerson, told the Columbia Basin Herald. “The safety of all our students is our top priority. Student safety is the number one job of our bus drivers.”

 

Cause of the Yakima County Crash

Lt. Scott Martin of the Washington State Patrol stated that the cause of the crash is under investigation. Roads were bare and dry when the bus rolled. No other vehicles were involved in the incident.

San Francisco Muni bus runs over bicyclist

A bicyclist was seriously injured Tuesday after he caught his wheel on a Muni track and fell under a bus, authorities said.

The bus, an outbound 31 Balboa, rolled over the man at the intersection of Market and New Montgomery streets at about 4:41 p.m. , said San Francisco Metropolitan Transit Agency spokesman Paul Rose.

He was transported to San Francisco General Hospital with life-threatening injuries, said Officer Albie Esparza, a police spokesman.

Traffic at the intersection was shut down pending the investigation, but opened again shortly before 6:45 p.m., Rose said.

Probe ordered into crash that injured 7 at 110 Freeway bus stop

LOS ANGELES —Seven pedestrians were critically injured Wednesday afternoon in a bus lane of the northbound 110 Freeway near the 105 Freeway when the car hit them, officials said.

The injuries range from serious to critical, according to authorities.

The accident was reported at 3:15 p.m., according to Erik Scott of the Los Angeles Fire Department.

All the victims were taken to hospitals.

According to a MTA spokesperson, the driver was going northbound on the 110 Freeway near a bus station. He entered the bus lane and realizing he entered the station swerved onto the platform in an effort to avoid hitting the bus.

Metro officials said Thursday they will launch an investigation into a harrowing crash at a South L.A. bus stop where seven people were stuck by a motorist.

Metro chief Art Leahy said at Thursday’s board meeting that he has requested a review that will examine the transitway, signage and other safety issues, and he wants a report within 60 days.

It was unclear what caused the driver to lose control of the vehicle. About 50 firefighters were called to the scene to help and assess the victims.

The accident prompted the CHP to issue a SigAlert for the busy artery just as the afternoon rush-hour commute was underway.

It is unclear if the driver was under the influence.  News Helicopters recorded footage of the suspected driver undergoing a sobriety field test conducted by CHP officers. The results of that test were not immediately revealed.
