Man dies in oil field water tank, another suffers broken ankle

A welder died and a superintendent was rushed to a hospital with a leg injury after a Wednesday morning accident inside a water tank in an Oildale oil field.

Authorities said the accident occurred when a temporary platform the two men were standing on gave way at about 9:30 a.m.

The welder, 54-year-old Barry Snelson of Oildale, suffered a head injury and the superintendent fractured his ankle, said Dean Fryer, a spokesman for Cal-OSHA, which is investigating the accident.

The superintendent was not identified. Fryer said both men were working for Brahma Group Inc., an engineering and contracting firm based in Salt Lake City.

Brahma could not be reached for comment.

While details remain sketchy and agencies on the scene provided sometimes contradictory accounts, Fryer said the two workers were testing the tank at the time of the accident.

“What they were doing was bringing water, pumping water into the tank to see where there may be leaks,” he said.

Kern County Fire Capt. Eric Coughran said the men were cleaning up after a welding repair job from the night before.

Coughran said the man with the leg injury was pulled out with a harness about an hour after the accident. He was taken to Kern Medical Center.

Shortly after, the other man was pulled out. He was then pronounced dead at the scene, Coughran said.

The tank measures 15 feet high and 10 feet wide with an 18-inch opening for entry and exit.

The tank is part of Live Oak Cogen, a 46 megawatt, natural gas-fired power plant located in an oil field about 1 1/2 miles west of South Granite Road. In addition to generating electricity, the facility sells steam for heavy oil production nearby.

Snelson’s widow, Diane, said he was a safety-conscious, honest family man who was “excited” to join Brahma earlier this year after being without full-time work for a few months.

“He loves the company that he works for, loves all the people that he was working with. They’re an awesome company,” she said.

Snelson had worked as a welder since the age of 18, she said, describing him as “very safety oriented and very easy-going gentleman.”

The couple have three children: Nicole, 30, living in Clovis; Julie 28, in South Carolina; and Anthony, 25, who lives with his parents.

Cal-OSHA investigating Arvin tank explosion that kills one, critically injures another

ARVIN — The explosion of a decommissioned crude oil tank in a vacant field just north of a stretch of businesses killed one man, critically injured another and left questions unanswered about what safety procedures the men used as they worked on the tank.

The men were using a torch to dismantle pipes on top of the tank when vapors inside ignited, blowing the 16,800-gallon container some 30 feet in the air and a distance of 79 feet, firefighters reported.

Arvin resident Cesar Martinez, 24, was carried some 50 feet into the air and more than 100 feet away, Kern County Fire Department Capt. Derek Tisinger said. Martinez died just after 9 a.m.

The other worker, identified by their employer Sky-Brand Services as 33-year-old Eric Robles, was thrown into a nearby chain-link fence.

The workers believed the tank was empty, Tisinger said.

The incident happened in the 300 block of Campus Drive, in a field a couple of hundred feet behind a Dollar Tree.

There was no resulting fire and no other people were injured, officials said. No evacuations were required.

Iram Rocha was in the area talking to an Arvin police officer when it happened. He said he saw a big piece of the tank fly and a man’s body fly through the air.

“You could feel it all over,” Rocha, speaking through a Spanish translator, said of the blast.

The person he saw had suffered obvious head trauma, Rocha said. Rocha was so shaken by what he observed that he walked to a patch of grass behind a nearby Burger King just to be alone and get his emotions under control.

Cal-OSHA investigators determined the two men were assigned to demolish the tank and that Martinez was removing pipe that was to be recycled as scrap metal, agency spokesman Peter Melton said.

“He was using a cutting torch on an enclosed pipe, and apparently gas might have escaped and it ignited,” Melton said. It was unclear who owned the tank and when it was decommissioned, he added.

Sky-Brand partner Dale Hill said Martinez and Robles were removing steel pipeline from the exterior of the tank. He said the company has done similar work on numerous oil tanks and hasn’t had problems.

“We do everything we can to make sure every precaution is being taken,” Hill said

In the oil industry, however, the common practice is to keep all flames at least 50 feet away from an oil tank, said Bakersfield oilman Chad Hathaway.

“Fire and gas, they don’t make a good combination,” he said.

Sky-Brand specializes in site cleanup and demolition work, removing scrap metal and selling it, Hill said.

He said he hadn’t yet been able to talk to the Martinez family, and they were being looked after by family and friends. He said the accident has left him and others devastated and the business is looking to see what the final investigation turns up.

Martinez had been employed by Sky-Brand for two years, Robles for six months, Hill said.

The tank’s operator of record was Davis-based Sun Mountain Oil and Gas, according to the California Division of Oil, Gas and Geothermal Resources. The division also reported that the accident took place in the H.S. Jewett lease in the Mountain View oil field.

Sun Mountain Oil and Gas could not be reached for comment Tuesday.

Arvin Police Chief Tommy Tunson walked around the blast area, surveying the scene. Arvin police provided traffic control and made sure residents didn’t cross the yellow emergency tape roping off the area.

Tunson described the incident as very tragic and he expressed his condolences to the family of Martinez and Robles. He said firefighters responded immediately and did an excellent job.

Police will not be involved in the investigation as it’s an industrial accident, he said.

“If the wrong thing is done I guess anything can happen,” Tunson said regarding the power of the explosion.

The explosion was not close enough to impact Arvin High School operations, but some kids who walk home on Campus Drive were rerouted, said John Teves, spokesman for the Kern High School District.

Fourteen county firefighters responded to scene, where they were assisted by the county’s Environmental Health Services Department, Hall Ambulance, Arvin Police, Pacific Gas & Electric Co. and a chaplain from Kern Medical Center, Tisinger said.

Cal-OSHA’s Melton said the agency’s investigation could take up to six months, during which time it hopes to find out exactly what the two men were doing, what safety precautions were taken, what regulations apply to demolishing such equipment and whether the right tools were being used.

Drowning deaths: Civic body slaps Rs 50,000 fine on contractor

A day after three brothers drowned in a pit dug along the Mithi river in Kalina, the Brihanmumbai Municipal Corporation (BMC) fined the contractor a meagre Rs 50,000. The civic body also sent a showcause notice to Shyam Narayan on Wednesday, asking him to explain the security lapses around the construction site in terms of proper barricading. BMC officials admitted that the security guards employed by the contractor were not present at the site on Tuesday since it was a public holiday.

While the Kurla police station had earlier named MMRDA officials in the FIR on Tuesday, on Wednesday it was made clear by MMRDA that the work was being done by the BMC. “Investigations showed that the BMC contractor is responsible for the negligence that claimed the lives of three children. We have now named the contractor in the probe,” said Deputy Commissioner of Police (Zone V) Dhananjay Kulkarni.

According to senior civic officials, contractors are responsible for maintaining security around construction sites. They are expected to adhere to the tenders conditions, whereby they are expected to adequately barricade the construction site and deploy security guards to monitor the area. Often, however, the tin barricades are stolen by local residents who sell them in the scrap market.

Chief engineer (storm water drains) L S Watkar said, “We have taken the necessary action by penalising the contractor. He has been asked to respond to the security lapses.”

“Anyone can easily trespass to the trench after crossing the premises of Kailas Parbat residential building at Kapadia Nagar. People from our slum have been regularly complaining to all agencies and the police department to put in place security measures since most of the children play here for lack of any other place,” said Azhar Chaudhury, resident of the area and uncle of the three children who drowned on Tuesday.

Eight-year-old twins Abdul Rahim and Abdul Karim and 12-year-old Abdul Aziz were playing in the open area adjacent to the river at Kapadia Nagar on CST Road when the incident took place. The pit was dug as part of the ongoing Mithi river project being undertaken by the BMC, under which retaining walls are being built to train the flow of the river.

Forklift accident kills laborer

A forklift accident killed a 32-year-old laborer yesterday in Mojave Desert community of Helendale where the victim was helping to line a waste water pond, San Bernardino County coroner’s officials say.

Jose Reyes died at the scene of the 9:14 a.m. accident Thursday along the 14400 block of Cougar Road, said coroner’s spokeswoman Sandy Fatland. Helendale is roughly mid-way between Victorville and Barstow.

The forklift was moving heavy rubber rolls that were being used to line the pond, Fatland said. It’s unclear exactly what happened because no witnesses saw the accident, she said.

The Santa Ana resident suffered a head injury, though he was wearing a helmet, Fatland said.

The death is being investigated by deputy sheriffs and Cal-OSHA.

Man badly hurt when thrown in rock crusher in Irvine

IRVINE – A construction worker suffered life-threatening injuries when he was thrown into a rock crusher at a construction site Wednesday afternoon, authorities said.

The incident was reported at about 12:50 p.m. at the 8700 block of Irvine Center, said Orange County Fire Authority Capt. Marc Stone.

Stone said it appears the man, who was roughly in his late 30s to mid-40s, was entangled in a conveyor belt and was dumped into a rock crusher.

The man yelled and screamed and his co-workers hit the emergency button before he went through, Stone said.

He suffered life-threatening head injuries and was taken to Mission Hospital, Stone said.

It took about 15 minutes for the Fire Authority urban search and rescue team to remove the worker, Stone added.

Man buried, dies when hillside collapses in Milpitas

 

When authorities arrived in a gated Milpitas hills community this weekend, they were expecting the worst: 911 dispatchers had reported a bad accident at the site where a construction crew was building a small mansion.

They found something even more gruesome than they could have imagined: a worker buried alive, crushed by an avalanche of soil. He was dead by the time rescue workers arrived, with only the top of his head visible.

Now police and state investigators are considering filing criminal charges against the contractor, who they say shouldn’t have been working at the site.

Just three days before Saturday’s deadly accident, city inspectors deemed the site was too dangerous and ordered the construction company to stop all work immediately. A combination of poor work by the contractor and the previous weekend’s rains increased the risk of the steep hillside collapsing, city officials said.

 

But the contractor, Fremont-based U.S.-Sino Investment, kept working on the new 5,800-square-foot house, anyway, city officials said Monday. A 37-year-old carpenter was doing wood work in a 12-foot ditch when it caved in, crushing him and killing him almost instantly, authorities said.

“It’s is one of those heartbreaking situations,” said Erika Monterroza, a spokeswoman for Cal/OSHA, the workplace safety agency that is investigating for the state.

Making this situation even more unusual, Monterroza said, was that it was initially too dangerous to remove the body. If officials jumped into the ditch, they would risk being buried, too, she said.

Late Monday evening, officials had finally pulled away the dirt and plywood and removed the body. But they have been unable to identify him.

Many questions remained unanswered Monday.

The Milpitas police and fire departments and theSanta Clara County coroner all referred comment to the city’s building department.

A man who answered the cell phone of Richard Liu, who is listed as the CEO of U.S.-Sino Investment, declined to comment and hung up on a reporter.

The city’s chief building official, Keyvan Irannejad, said authorities won’t know until Tuesday at the earliest whether criminal charges will be filed.

Irannejad said the workers had previously cut a 12-foot-deep hole at the site to build the house’s foundation. But they didn’t put any system in place to “shore” the hole to ensure the trench would not collapse, Irannejad said.

After city building inspectors visited the site Wednesday, they issued a stop-work order until U.S.-Sino could prove they had shored the trench. Anyone found violating the order can be fined $1,000, but the city had not gone back to enforce it.

“They just ignored our stop-work notice,” he said. “When we got there (after the accident), we didn’t know they’d been working.”

Irannejad said Milpitas has never worked with U.S.-Sino before the permit was issued for the house in August. Monterroza said the state has no record of violations at the company, which began operating in 2008.

U.S.-Sino could face fines, and both police and Cal/OSHA could recommend prosecutors file criminal charges.

Worker Dies In Palm City Construction Accident

SAN DIEGO — A 58-year-old construction worker was killed Tuesday when the aerial-platform machine he was operating toppled over at an equipment yard, authorities reported.

The fatal accident occurred as the man was unloading the boom lift — a type of apparatus sometimes called a “cherry picker” — from a flatbed truck at the lot in the 800 block of Hollister Street in the Palm City neighborhood shortly before 11:30 a.m., according to the San Diego Fire-Rescue Department.

The machine tipped over and fell to the ground after one of its wheels slipped off the the edge of the truck while the worker operated it from inside its extendable basket, SDFRD spokesman Maurice Luque said.

The man, who would have turned 59 on Saturday, died at the scene. His name was withheld pending notification of his family.

The state Division of Occupational Safety and Health, or Cal/OSHA, will investigate the fatality.

Thousand Oaks man reportedly killed in Santa Barbara showgrounds accident

A Thousand Oaks man reportedly was killed Monday in Santa Barbara as he dismantled equipment used during a rodeo at the Earl Warren Showgrounds last weekend, a Santa Barbara County Fire Department spokesman said.

A 1,500-pound monitor fell on the man around 1 p.m. When firefighters arrived, they found workers had moved the screen off the man, who was unconscious and not breathing, said Brian Hayden, an engineer and inspector with the Santa Barbara County Fire Department.

“CPR was performed on him on the way to Cottage Hospital, which is just the short distance away,” Hayden said.

The man’s identity could not be confirmed, but he reportedly was a Thousand Oaks business owner.

The accident that occurred on August 8th remains under investigation by authorities. However, based on the limited information provided by KSBY and KYET, it is unclear why the  1500lb. Jumbotron LED screen fell on Mann. Hopefully authorities can come to a conclusion of what happened and why.

Beaumont water district worker dies in crash

An employee with the Beaumont-Cherry Valley Water District died in a work site crash Tuesday in Beaumont.

Ricardo Flores, 24, of Beaumont was pronounced dead at the scene on Veile Avenue near Sixth Street at 2:44 p.m., Riverside County coroner’s officials said.

Flores was marking the street when he was struck by a stake bed truck. Beaumont police are investigating the crash.
