Fatal Traffic Collision in Hesperia
On April 23, 2013 at about 2:50 p.m, Deputies from the Hesperia Station and units from the California Highway Patrol along with rescue personnel from the San Bernardino County Fire Department responded to a report of a two vehicle, head on traffic collision in the area of Hwy 138 and Little Horsethief Ranch Rd. in Hesperia. It was reported that the two vehicles had collided head on and that there were serious injuries to several parties. Rescue personnel arrived on scene and discovered that the male driver of a 1991 Ford Explorer was trapped inside his vehicle and suffered fatal injuries upon impact. The driver was later identified as Robert E. Miller, age 57 of Victorville. A male passenger from that same vehicle was suffering from injuries, not life threatening.
Three occupants from the second vehicle, a 1999 Toyota minivan were also seriously injured, one with life threatening head injuries. Rescue personnel extricated the victims from their vehicles and all were transported to Arrowhead Regional Medical Center by both air and ground transportation.
The Hesperia Station Major Accident Investigation Team (MAIT) responded to conduct the investigation. It was determined that for unknown reasons the driver of the Ford Explorer who was traveling east on Hwy 138 at an undetermined speed, crossed over into the westbound lanes and collided head on into the Toyota minivan as it traveled west on Hwy 138.
Hwy 138 was closed in both directions between Summit Valley Rd. and Hwy 173 until about 1930 hours while investigators processed the scene for evidence. At this time the exact cause of the collision and why the driver of the Ford Explorer crossed into the westbound lane is still under investigation by the Hesperia Station Traffic Division MAIT Investigators.

Cyclist suffers major head trauma in crash with car

Crash with Victorville sheriff’s vehicle injures four children
Four children were injured Halloween night after the vehicle they were traveling in was involved in a crash with a Victorville Sheriff’s patrol unit, San Bernardino County Sheriff’s officials said Thursday, and some witnesses say the patrol unit did not have its lights and siren on when the collision took place.
Initial information from the sheriff’s department does not indicate the unit had its lights on, but officials couldn’t immediately comment if the deputy ran a red light leading to the crash.
The children were airlifted to Loma Linda University Medical Center with moderate injuries and two others were taken to local hospitals following the crash which took place in the intersection of Bear Valley Road and Seventh Avenue, according to Cindy Bachman, spokeswoman for the sheriff’s department.
Around 9 p.m. Deputy Vanessa Duardo was traveling east on Bear Valley Road to an attempted armed robbery at the Goodwill store on Bear Valley and Hesperia roads, officials said, when her vehicle collided with a 1989 Chevrolet Blazer traveling north on Seventh Avenue.
The impact caused the Blazer to roll over ejecting the two children who witnesses said to be about the age of 5.
Duardo was taken to a local hospital with minor injuries, officials said. She is not expected to immediately return to work.
Dozens of witnesses, some children still in their Halloween costumes, gathered at the intersection of Bear Valley Road and Seventh Avenue and watched the scene unfold.
Cause of the Victorville Pedestrian Crash
The cause of the October 31, 2012 accident remains under investigation by the San Bernardino County Sheriff’s Department.

CHP Reports Alcohol played role in fatal Willow Creek bus crash
An unidentified 19-year-old woman was killed and seven other people were injured when a bus drove off a steep embankment on State Route 96 late Friday night, according to a California Highway Patrol press release issued Saturday.
Around 9:50 p.m. a 1993 GMC former school bus driven by Lorenzo Jackson, 42, of Washington state, drove off the right of the highway for unknown reasons approximately four miles north of State Route 299, according to the release. The bus rolled a short distance down a steep embankment, and came to rest against some trees.
Jackson broke his leg exiting the bus, according to the release. Two other passengers exited the bus and remained nearby while two passengers walked up to State Route 96. Three people remained in the bus, including a woman who was pronounced dead at the scene by paramedics.
Deputy Coroner Roy Horton said the woman’s identity is currently unknown, and that the passengers on the bus did not know very much about each other.
During investigation of the crash, it was determined that both Jackson and 35-year-old Joseph Forcier of Florida had driven the bus under suspicion of alcohol intoxication prior to and during the collision, according to the release.
Both men were arrested and remain at medical facilities to be treated for their injuries. Forcier received major head trauma and was transported to Mad River Community Hospital, according to the release.
Five other accident victims were transported to St. Joseph and Mad River Hospitals. The deceased victim’s name has not been released.

Two dead in crash on I-5 at Tejon Pass
GORMAN — A sport utility vehicle that may have been carrying a family back from the Labor Day holiday weekend crashed Tuesday on the Grapevine section of Interstate 5 north of Los Angeles, causing a chain-reaction pileup that killed a woman and a young girl and injured nine others, officials said.
The critical mountain pass between Southern and Central California was jammed for a mile or more after the accident that shut down southbound lanes at around 5 a.m., California Highway Patrol officials said.
A multi-car pileup in the Grapevine stretch of southbound Interstate 5 in Gorman killed two people, injured nine others and created a mile-long traffic jam early Tuesday morning.
A 41-year-old woman driving the SUV southbound hit the center divider in the Gorman area, hurling her and an 11-year-old girl out of the car. They were pronounced dead at the scene, CHP Lt. Craig Whitty said. It wasn’t known whether they were wearing seatbelts.
The pre-dawn accident spawned others as cars hit wreckage and each other or spun out trying to avoid crashing.
“From that point on, there are at least four more passenger cars and at least one big-rig involved in continuous collisions,” Whitty said.
Four people were critically injured and five received minor injuries, Los Angeles County fire officials said.
Their identities were not released but the injured included a grandmother in the SUV and three girls, ages 2, 12 and 16, authorities said.
Officer Adriaan Garcia, of the CHP’s Fort Tejon office, said the cities of residence of the victims were not yet available. He said officers believe all of the children were traveling in the SUV.
“This will take a while to decipher,” Garcia said of the crash.
Three people taken by ground ambulance to hospitals were a 29-year-old woman, 50-year-old woman and 48-year-old man, Garcia said.
Southbound lanes were shut for more than two hours so helicopters could land and take away the injured. A shoulder finally was opened so that thousands of cars could begin creeping through the pass. All lanes later reopened.
Morning commuter traffic may have been swelled by people returning from Labor Day vacations.
“I did see a lot of people towing trailers,” Whitty said.
The holiday also may have played a role in the crash, although the cause remained under investigation.
“It was a family loaded in the car. We think they were probably coming back from a long holiday weekend,” Whitty said. “At this point we’re thinking the driver may have been fatigued.”
Interstate 5, one of the state’s major north-south corridors, carries a heavy mix of truck and car traffic over a mountain pass rising to an elevation topping 4,100 feet about 50 miles north of Los Angeles.

Fremont woman hit by a car in crosswalk in critical condition
A Fremont woman hit by a car Tuesday while walking in a crosswalk on Civic Center Drive remains in critical condition.
Manchi Wong, 59, suffered a major head injury after being struck around 5 p.m. in the 38900 block of Civic Center Drive, said Sgt. Mark Dang.
Wongwas unresponsive at the scene, police said, and taken to Eden Hospital. She remained hospitalized and unconscious on Wednesday morning, Dang said.
An early investigation indicated 19-year-old Nathaniel Fedalizo, of San Jose, was driving a sedan north on Civic Center Drive and that Wong was struck while walking west across the road in the crosswalk, police said.
Fedalizo was uninjured, and police said drugs or alcohol do no appear to be a factor.
As of Wednesday no charges had been filed against him, Dang said.

Baby blinded by shaking; Madera caretaker arrested
A 34-year-old Madera woman was arrested Tuesday night on suspicion of shaking a 4-month-old boy, causing what doctors said are permanent blindness and a loss of hearing to the infant, police said.
Natividad Marcus Reyes, 34, the boy’s baby sitter, was taken into custody about 7:15 p.m. at her home in the 1400 block of Austin Street, Madera police detective Sgt. Dino Lawson said.
Reyes faces felony charges of inflicting injury on a child and willful cruelty to a child, Lawson said.
Detectives began their investigation when they were called to Children’s Hospital Central California on July 25 for a suspected child abuse case, Lawson said. Doctors said the infant suffered a preretinal hemorrhage, Lawson said.
Reyes took care of the boy while his mother — Rosalia Gonzalez, her next-door neighbor — worked. During the investigation, Reyes confessed to police that she had shaken the baby, Lawson said.
Reyes’ four children, all 10 years old and younger, who live with her have been placed in protective custody, Lawson said.
In 2009, Reyes was charged with child abuse involving a fifth child of hers, Lawson said. She completed a parenting program and the case was dropped. That child now lives in Mexico.

A head-on collision Monday morning north of Huron left one person dead and five people with major injuries, the California Highway Patrol reported.
Joginder Singh, 68, of Fresno, died at the scene.
Singh was a rear passenger in a 2004 Honda that collided with a 1997 Chevrolet on Highway 269 south of Oakland Avenue about 5:10 a.m.
According to the CHP, the driver of the Honda, identified as John Doe, was headed south, but had allowed his vehicle to veer into the northbound lane, directly into the path of the Chevrolet driven by Lorenzo Salazar, 23, of Avenal.
Doe and two other passengers in the Honda — Rahjit Singh, 35, and Balhara Singh, 58 — suffered major injuries, the CHP said. Salazar and his passenger, Lorenza Bautista, 46, also suffered major injuries. They were taken to Community Regional Medical Center in Fresno.
“Drugs and or alcohol do not appear to be a factor in this collision,” a CHP
news release said.

Girl Killed, Mom and Sister Critical in Santa Ana Hit-and-Run
SANTA ANA, Calif. (KTLA) — A woman and her daughter are still listed in critical condition after they were hit by a suspected drunk driver in Santa Ana on Saturday.
The accident happened about 11 a.m. near the intersection of 17th and Spurgeon streets.
The driver, Jessica Louise Cowan, 33, of Frazier Park, hit three pedestrians — a 45-year-old Eloisa Madrigal, her 5-year-old daughter Osmara Meza and her 6-year-old daughter Grecia Meza — before speeding away from the scene, police said.
A witness told investigators that Cowan ran a red light.
A preliminary field sobriety test showed she was under the influence of alcohol or drugs.
Medical marijuana was also discovered in Cowan’s vehicle.
She faces charges of drunk driving, hit-and-run and vehicular manslaughter.

Bicyclist badly hurt in S.F. crash
A bicyclist suffered life-threatening injuries when he was hit by a truck Tuesday in San Francisco, police said.
The accident happened at Franklin and Oak streets about 2:30 p.m. when the truck driver stopped at a blinking red light, then made a right onto Franklin. The truck hit the bicyclist, who was riding on Oak in the opposite direction and had a green light, said police spokesman Officer Carlos Manfredi.
The driver stopped and cooperated with officers, Manfredi said. The bicyclist was taken to San Francisco General Hospital.
